New patients can save time during their first appointment by completing the New Patient Information prior to their visit. Print out all three forms, fill in the information requested, and bring the completed form with you to your appointment.
Registering for school? Parents have the option to print out and prefill the patient information prior to the appointment. We also offer the form in the office
Need a sports clearance? Select the school district that your child attends, fill in the health history portion, then book an appointment for your child to be cleared by a physician
We are a pediatric primary care provider for the Fresno and Clovis area. We provide multiple pediatric services for your child's healthcare. We are also available for walk-ins during our regular business hours at any of our locations.
